Following identifiers must be included:
- Ordering Provider's NPI
- Driver's License Number (9-digits)
NOTE: A NYS Driver's License Number must be reported as 9 digits (numeric characters). Out-of-state Driver's License Numbers must also be sent as 9 digits. If the out-of-state license is more than 9 digits send only the first nine digits. If it is less than 9 digits it may be zero filled on either the right or left to make a total of 9 digits. Eliminate any alpha characters in the license.

The intended audience for this guidance is providers who fall under the following Categories of Service (COS): 0602, 0603. 0604, 0605, 0606, and 0609.

» The ORDERING Provider involved in the Non-Emergency Transportation Services must be identified.
The claim must include the Ordering Provider information in the Referring Provider Name (Loop 2310A, NM1 Segment), with the following values:
NM101 will contain "DN", Entity Identifier Code
NM108 will contain "XX", Identification Code Qualifier for NPI
NM109 will contain the Ordering Providers NPI
NOTE: If the referral came from a Clinic, then a Physician on-staff at the Clinic must be identified.
» The Driver's License Number must be included in the claim in the Referring Provider Name Secondary Identification (Loop 2310A, REF Segment). The following values are expected in the Referring Provider Name Secondary Identification REF segment:
REF01 will contain "0B", Reference Identification Qualifier
REF02 will contain the driver's license number of the vehicle operator
NOTE: Drivers license MUST be 9 digits. If the license is more then 9 digits, limit the entry to the first 9. If the license is LESS then 9 dicits the entry must be right justified and zero-filled to complete the 9 digits. If the reported license is not exactly 9 digits, submitters will hit eMedNY Edit 00267 Vehicle License Plate / Driver's License Required. The HIPAA CODES are CARC 16 and RARC N291.
» The RENDERING Provider involved in the Non-Emergency Transportation Services must be identified.
The claim must include the Rendering Provider information in the Rendering Provider Name (Loop 2310B, NM1 Segment).
NM101 will contain "82", Entity Identifier Code
NOTE: This NM1 Segment contains the Non-Emergency Transportation provider. Some transportation providers have both Non-Emergency Services and Emergency Services, and they have an NPI, however, when billing for Non-Emergency Transportation the NPI must not be sent.
» The License Plate Number must be entered in the Rendering Provider Secondary Identification (Loop 2310B, REF Segment). The following values are expected in the Rendering Provider Secondary Identification REF segment: REF01 will contain "G2", Reference Identification Qualifier
REF02 will contain the license plate number.
» ADDITIONAL REQUIREMENTS: If the Passenger (Patient) is a Restricted Recipient the Passenger's Primary Care Provider Identifier is required, and the claim must include a second iteration of the Referring Provider Name (Loop 2310A, NM1 Segment), with following values:NM101 will contain "P3", Entity Identifier Code
NM108 will contain "XX", Identification Code Qualifier
NM109 will contain Primary Care Provider's NPI
For ePACES Professional claims:
The Driver's License Number and License Plate Number can be reported in the Professional Claim Information Tab if all the service dates have the same Driver License number and License plate number. If the service dates have a different Driver License number and/or License plate number, this information can be entered in the "More" option on each service line.
The Ordering Provider's information should be entered in the Ordering Provider section unless the passenger is a Restricted Recipient (for a Restricted Recipient, enter the National Provider Identifier (NPI) of the provider to whom the recipient is restricted in the Primary Care Provider field.
The ordering provider can also be entered on the Service Line Tab in the "More" option.
For Paper Claims:
The License Plate Number should be entered in the Service Provider field (field 21) with NO profession code.
The Driver's License Number (9-digits) should be entered in the Other Referring Provider ID field (field 22).
